The filing requirements for Accredited Agents and Attorneys are found in 38 CFR § 14.629 (b) (iii) and (iv)
The VA does not mandate that accredited agents and accredited attorneys MUST use the forms provided below. The certification letters sent to the Office of General Counsel can be in any form, as long as the required information is provided. However, the Office of General Counsel has approved the attached sample forms, and they MAY be submitted.
The forms on the links below may assist accredited agents and accredited attorneys in fulfilling the VA's requirement that those who are accredited will certify in writing to the Office of General Counsel that they have completed the post- accreditation CLE requirements. A "Certificate of Completion" for any CLE course taken does not need to be submitted with the report.
